The Office of the Chief Information Officer has issued NOFA #004 for the Empower Rural Iowa Broadband Grant Program making available up to $4,950,000 for broadband expansion in Iowa. The application window for this program closed November 22, 2020 at 5:00 CDT.

NOFA 004 section 1.7.6.1.2.1. requires "Signed attestations submitted to the Office
under penalty of perjury on forms supplied by the Office, attesting that a Targeted Service Area(s) forming the basis of a Project is currently served with 25/3 Broadband or 100/20 Broadband, whichever is applicable." Therefore, you must also submit an attestation form. This form can be downloaded via the link below. Further, NOFA 004 section 1.7.6.1.2.2. allows for "Bills or invoices provided to or received by customers in a Targeted Services Area(s) forming the basis of a Project which identify current Broadband service at or above 25/3 Broadband or 100/20 Broadband, whichever is applicable."
